Annual Notifications and Forms

Each fall, returning students' parents and guardians must review their contact information, give permission for things such as walking field trips, and acknowledge receipt of important policies. This process is done directly in PowerSchool. If you don't remember your parent PowerSchool account login information, please contact a secretary at the building your student attends.  Instructions for completing the forms can be found here.


2025-2026 Student Surveys

MiPHY (Grades 7, 9, 11) Students in Grades 7, 9, and 11 will participate in the Michigan Profile for Healthy Youth Survey supported by the Michigan Department of Education during the last few weeks of April. The research survey will ask about the health behaviors of 7th, 9th, & 11th-grade students. You can preview the questions for the MiPHY Survey here: 

Student Perception Survey, this annual student perception survey is designed to gather feedback about students' experiences at school. It measures student perceptions of school safety, academic support, communication, engagement, social-emotional well-being, mental health resources, and equitable access to learning opportunities. The results help the school identify strengths, monitor progress, and inform continuous improvement efforts.

  2. I know the learning targets or skills for all my classes.
  3. My parents help me with my schoolwork at home.
  4. I know what events are happening at school.
  5. My teacher and I talk about how I'm doing in school.
  6. I feel okay asking for help and sharing feedback with my teacher.

  8. I know how to monitor my own behavior progress.
  9. I attend class and participate every day.
  10. I have opportunities to work with my classmates to complete work.
  11. I feel emotionally supported at school.
  12. There are staff members I can approach if I need mental health support.
